Welcome to the Community Healthcare Services On-line. Polk County has a long history of providing emergency services to its citizens to meet their basic needs, both health and social, when there is no other resource available to them. The Community Healthcare Services Division is comprised of several different service elements which focus on serving both the social and health care needs of the residents of Polk County.
